I collect personal information necessary to provide effective therapeutic services. The information I may collect includes:

  • Personal Identifiable Information: This may include your full name, contact details (email, phone number, address), date of birth, emergency contact information and GP contact information.
     

  • Health Information: To tailor therapeutic services to your needs, I may collect relevant health information, including medical history, psychological history, and information related to the issues you're seeking therapy for (e.g., stress, anxiety, anger, sleep problems).
     

  • Session Records: Information shared during therapy sessions, including treatment plans, progress notes, and any goals you set, are kept secure when not in use.
     

  • Payment Information: If applicable, billing details for payment purposes.

How your information is used 

I use the information you provide for the following purposes:

  • To Provide Therapy: I use your information to assess, diagnose, and tailor our sessions to meet your needs and goals.
     

  • Administrative Purposes: To schedule appointments, communicate session reminders, and process payments.
     

  • Improvement of Services: To evaluate and improve the quality of therapy I provide and for general administrative purposes, such as training or supervision, which is always done in a confidential and annonymised manner.
     

  • Legal Compliance: To comply with legal and ethical obligations, including safeguarding, reporting, and record-keeping requirements for licensed therapy professionals.

  • Risk management: If I am concerned about your safety, I may need to contact your GP without your consent, but I will always try to get this before.

Confidentiality 

The BABCP Standards of Conduct, Performance and Ethics require that I maintain your confidentiality and keep information about you safe and secure.

  • Your Consent: I will only disclose information to others with your consent. For example, if you would like us to communicate with your doctor, spouse, or another professional.
     

  • Legal or Ethical Obligations: In certain circumstances, I am required to disclose information without your consent, including:
     

    • If I believe there is a risk of harm to yourself or others.

    • If required by law, such as in cases of child abuse or neglect, or when ordered by a court.
       

  • Supervision and Consultation: To provide the best care, therapists may discuss cases with other licensed professionals in a confidential and anonymous manner. These discussions are for the purpose of improving treatment and ensuring ethical practice.

How your information is stored

  • I have my own professional liability insurance and am a registered data controller under GDPR with the Information Commissioners Office (ICO).

  • Session notes and therapy resources , including questionnaires when applicable or recordings, are kept securely, in accordance with the Data Protection Act (2018) the UK General Data Protection Regulations (GDPR).

  • Under Common Law I keep records for six years.

  • I take reasonable steps to protect the confidentiality of your personal information. We have implemented appropriate administrative, technical, and physical safeguards to prevent unauthorised access, disclosure, alteration, or destruction of your personal data. These measures include, but are not limited to:

    • ​Secure storage of physical records in locked, access-controlled areas.

    • Encryption of electronic records and secure data storage.

Your rights

You have the right to:
